HIRING: RN Case Managers for a HYBRID opportunity in Baltimore, MD

Shift: Full-time, 40 hours per week Mon-Fri, must complete minimum of 17 assessments per week.Location: Hybrid around Baltimore city limits

Pay: Per assessment, mileage is reimbursed between appointments

Start Date: June 10th or 11th

Duration: Indefinite contract, no end date!

Qualifications:

- Holds a valid State of Maryland RN license

- Has 2 years of experience in medical-surgical/geriatric and/or pediatric case management

- Own vehicle, driver's license and auto insurance

- MUST BE WILLING TO TRAVEL TO CLIENT HOMES THROUGHOUT BALTIMORE CITY MARYLAND

Job Description:

- Conduct comprehensive assessments: Analyze medical and psychosocial information, conduct home visits, and evaluate client needs to identify appropriate services.

- Will be using the LTSS EMR system

- Develop personalized care plans: Help clients access home and community-based services that empower them to maintain independence in the least restrictive environment.

- Coordinate and connect: Refer clients to resources, consult with professionals, and coordinate services to ensure a seamless support system.

- Champion client autonomy: Advocate for client rights, promote self-determination, and ensure fair access to vital services.

- Document for impact: Maintain accurate records to track progress, inform service decisions, and contribute to the ongoing improvement of the AERS program.
